Assistance for persons experiencing homelessness obtain their birth certificate and State of Florida identification card.

Birth Certificate:

An original birth certificate can be obtained by written request to the Bureau of Vital Statistics in the county where you were born. There is usually a fee for this service. To find the address and phone number of the Bureau of Vital Statistics for the county of. your birth, look in the local library for the directory of the city in which you were born. You can also phone information for H1etelephone number. Dial 1(area code of the birth county)555.1212 and ask for the Bureau of Vital Statistics. This is usually a free call.

Social Security Card:

Identification Card:

To get an identification card, you can apply for one at any driver's license office. You must:
1. Be 12 years of age or older.
2. Present your official Social Security card.
3. Have two items of identification, one of which must list your full name and date of birth, and a second supporting document, such as: birth certificate, school record stating date of birth, baptismal certificate, family bible record, passport, Florida vehicle registration certificate, court order which includes name, voter registration card, marriage license or certificate, Florida or out-of-state driver license, military ID card.
4. There is no fee for Florida homeless. You must present documentation of homelessness.
