INSTRUCTIONS for Online Submittal of UTILITY FRANCHISE Permit Package:

**FIVE (5) MINIMUM SUBMITTAL REQUIREMENTS.**

1. Site Plan showing ALL proposed work. (No scanned drawings. If there is an instance where only a scanned image can be used, ensure the resolution is 300 DPI or higher. Documents must be legible on a computer screen and/or printed.)
2. Utility Permit Application (on latest form-see 'Apply for A Permit' below).
3. Utility Checklist (on latest form-see 'Apply for A Permit' below).
4. Copy of City Utility GIS Atlas Maps/As-builts (Water, Sewer, Storm) in vicinity of proposed work.
   • Contact plan@fortlauderdale.gov to request
   • Show and Label in both plan and profile views
5. User Registration Form (Information to be current per Florida Ch_2019-131)

We will accept e-permit applications from City-Registered Utility/Agency Owner (UAO) and Contractors only.

Once your E-Permit application is received by DSD-Engineering Services, it will be processed on a "first-come, first-served" basis. When we have completed the processing, you will be notified via email.

When submitting corrections/revisions, do not resubmit the entire plan set. Only upload the sheets containing changes from the previous submission. Make sure page numbering is EXACTLY the same as the original sheet it is replacing. (select the magnifier glass to select the sheet to replace from the dropdown list)

INSPECTIONS and PERMIT CLOSEOUT

REQUIRED INSPECTIONS:

An ENG-ENGINEERING ROUGH inspection will be scheduled automatically for you upon Permit Issuance.

PERMIT CLOSEOUT PROCESS:

APPLICANT TO UPLOAD: As-built drawings to the Engineering Plan Reviewer via a Modification Request:

Include the following:

1. Adobe PDF Format of associated project Bore Logs

2. Restoration photos 

3. Plot of Constructed Bore (utilizing Bore Logs) against the Approved Bore Design at permitting (to ensure there are no utility conflicts).

Note: If as-built is Disapproved, City Reviewer will post comments to the record to be responded to by Applicant.  If as-built is Approved, City Reviewer will schedule the final engineering inspection based upon inspectors' availability. 

**Please do not upload as-built drawings to the permitting system or schedule the final inspection.**
